About Us 

Mosaic Primary Care Network's vision is "Better Health for All".

Our vision honours the area’s diversity through its provision of inclusive primary care. We recognize the need to consult and respect the health care goals and aspirations of each local community group within their ethnic framework.  We also realize the need to support and maintain the health of physicians and other health care professionals working in the Mosaic PCN area in order to maximize their contribution and working life.

The principles of Mosaic PCN are to:

  1. Ensure transparency
  2. Promote work-life balance for all health care providers
  3. Foster cultural competency among staff
  4. Remain grassroots and patient-centred
  5. Enable the execution of decisions by the governing body
  6. Respect physician engagement and leadership
  7. Provide seamless care which avoids duplication of AHS services

Objectives

Mosaic PCN adopts the following provincial health care objectives to:

  1. Increase the proportion of residents with ready access to primary care
  2. Provide coordinated 24 hour, 7-day-per-week management of access to appropriate primary care services
  3. Increase the emphasis on health promotion, disease and injury prevention, care of the medically complex patient, and care of patients with chronic disease
  4. Improve coordination and integration with other health care services, including secondary, tertiary, and long-term care through specialty care linkages to primary care
  5. Facilitate the greater use of multidisciplinary teams to provide comprehensive primary care

The Mosaic PCN Catchment Area

The geography of the Mosaic PCN includes urban Calgary east of 14th Street W, from the Bow River to the east city limits, and from south of the north city limits to Anderson Road/114 Ave SE.

As shown on the map to the right, the Mosaic catchment area is outlined in purple. The population of this area is approximately 260 000 people.
