Under this initiative, Mosaic PCN aims to improve community members’ access to primary care within the Mosaic catchment area (see About Us). Mosaic PCN recognizes that the population it serves contains significant cultural, ethnic, and socioeconomic diversity, making access in this area a unique and challenging issue. As roughly 50% of community members within the catchment area do not have a family physician, Mosaic PCN considers connecting individuals to primary care of utmost importance.
Need a Doctor?
Mosaic PCN also works to connect patients without a family physician to a Mosaic physician accepting new patients. This is done through a web registry found at http://www.needadoctorcalgaryandarea.ca. In addition to helping patients find physicians, Mosaic PCN is committed to providing community members with continuity of care, meaning seamless access to all health care providers necessary to resolve their health problems and questions.
Services:
One way Mosaic PCN achieves this is through the Mosaic Primary Care Clinic which offers a medical home to individuals in this area without a family physician. The multidisciplinary team (MDT) working at this clinic assists with creating continuity of care for patients with complex chronic disease and multiple health issues. Outside of regular business hours, Mosaic PCN also operates an After Hours Clinic which receives patients with non-emergency medical issues as determined by a Health Link Alberta nurse. As part of the Community Cultural Engagement program, Mosaic PCN employs several strategies to reduce cultural and language access barriers. One strategy is the creation of a Resource Navigator who connects community members with organizations, programs, and resources according to their particular needs. The Resource Navigator also makes efforts to advertise the PCNs clinics and services in various cultural newspapers and radio stations. Finally, Mosaic PCN uses interpreters, who attend clinics when they are needed, and Language Line, where an interpreter can be accessed by the clinic over the phone. Mosaic clinics and affiliated clinics set up these services on behalf of patients who are not fluent in English.
