
In 2008, diabetes was the 6th most common reason for primary care visits in Mosaic PCN’s catchment area. Diabetes is generally a poorly managed disease that creates a substantial burden on acute care providers, like hospitals.
Currently, the Diabetes, Hypertension and Cholesterol Centre (DHCC) in Calgary is located at Richmond Road Diagnostics and Treatment Centre in the city’s southwest. Mosaic PCN is aware that access to this facility is a challenge and a barrier for many patients within the Mosaic boundaries.
It is anticipated that group insulin starts and education will be offered by the DHCC in partnership with Mosaic PCN.
