Mosaic PCN has five stated priority initiatives which it addresses through the programs and services delivered under each initiative:
Access and Continuity of Care
Includes the Mosaic Primary Care Clinic, the After Hours Clinic, and Community Cultural Engagement.
Chronic Disease Management (CDM)
Includes Chronic Disease Management Nurses, Physician support for CDM participation, and Physician case management of Complex PLC (Peter Lougheed Centre) Acute Care Patients; coming soon will be the Community Multi-Disciplinary Clinic, the Cardiac Rehabilitation Program, and the Chronic Pain Clinic.
Mental Health
Includes Mental Health Counsellors, Shared Mental Health, Group Mental Health Sessions for anxiety and stress, the Refugee Mental Health Clinic, and the Culturally Sensitive Mental Health Program.
Prevention and Promotion
Includes a Childhood Obesity Dietitian, an Oral Hygienist, the Seniors’ Fall Risk Assessment, Professional Development, Tobacco Cessation Programs (including classes for women of childbearing age), the Lift the Lip Oral Health Screening and Referral Program, and the Early Child Caries Fluoride Varnish Program; coming soon will be the Ronald McDonald House Charities Sponsored Mobile Unit.
Women’s Health
Includes the Mosaic Women’s Health Clinic with a low-risk maternity group, Nurse Practitioners, a Prenatal/Women’s Health Dietitian, and Centering Pregnancy.
